- You’ll first be asked to enter your email address. Make sure you enter it correctly (spelling, etc), as it will be sending you a confirmation link to your inbox first.
- If for some reason you click the link you receive in your inbox, and then accidently close that window, you’ll need to come back here, click the link above again, and restart the process.
- Once you’ve completed it & signed it, we’ll both receive a copy for our records.
